Occupational Therapist, £29.56ph, Reablement, Worcestershire
Pertemps are recruiting for a qualified and experienced Occupational Therapist to join Worcestershire County Councilâs reablement service.
This post will support the Reablement Team focusing on the community reablement aspect; assisting patients requiring additional support and rehabilitation who need increased care or therapy input following referrals made through to the area social work teams. Assessments are holistic and focused on maximising independence at every stage, ensuring best use of limited care funds.

Duties:
  • Undertake Occupational Therapy and care assessments and provide therapeutic intervention for a designated caseload, working closely with individuals to identify goals as part of their overall care plan with the aim of increasing independence and minimising care requirements.
  • Provide case management support for a designated caseload, with guidance from Senior Occupational Therapists
  • Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, individuals, their carers as well as all other services.
  • Enable individuals to achieve maximum independence with activities of daily living, providing appropriate direction and guidance to Therapy/Promoting Independence assistants in the service to support an individualâs rehabilitation and reablement where able.
Essential Requirements:
  • Substantial experience in Occupational Therapy, including assessment and working with both individuals and carers to improve their well-being.
  • Significant post registration experience in a health and/or social care setting.
  • Substantial experience of joint working with other agencies and disciplines.
  • Considerable experience of working with adults with physical disabilities and/or long-term conditions.
  • Significant experience in prioritising and managing caseloads
  • Significant experience of delivering manual handling training to informal and formal carers.
