About the role
Area role covers:
- Job 1 - Kettering and Corby area - 35 hours
- Job 2 - Northampton area - 35 hours
You will have the rewarding experience of providing information, guidance and support to people affected by dementia in identifying their needs, to help maintain independence, improving sense of well-being, and putting them in more control of their lives.
This role will provide dementia knowledge and expertise within one of four locality Welfare Support Teams as part of the Integrated Care Across Northamptonshire. (ICAN) Ageing Well.
You will:
- Be the ‘go to’ person for all that is dementia related.
- Work closely with health, adult social care and third sector colleagues assessing, signposting, and providing information and guidance to support people affected by dementia.
- Build robust working relationships and develop networking opportunities with a range of local partners and community organisations.
- Create pathways with partner organisations who work in the field of dementia, ensuring seamless, holistic support and empowering individuals to make informed choices.
- Provide dementia support face-to-face, by telephone and online.
- Work closely with the Age Well teams and Memory Assessment Service.
- Facilitate our range of face to face and virtual peer support groups, deliver a Carers information support programme and attend information events as needed.
About you
We are looking for an exceptional individual, with an understanding of memory loss, dementia and the needs of those affected with these conditions.
You will:
- Be able to assess client needs
- Understand the need for client confidentiality
- Have excellent non-judgmental communication
- Have good organisational and time management
- Evidence good IT skills and experience working with databases
- Have demonstrable experience.
- Have an empathetic approach and a commitment to equal opportunity
- Be able to travel independently across Northamptonshire.
As a community-based worker, you will be required to attend a range of meetings, appointments and venues as well as working from home. A DBS disclosure will be required.
