Main duties of the job

Duties include the management of all aspects of patient care in the critical care environment as part of multidisciplinary team. The posts will be designed around the specific requirements of the doctor, with particular attention paid to progressing the towards a goal of achieving CESR.

Additional training blocks can be provided in neuro-intensive care, cardiac intensive care and anaesthesia as required. Depending on availability, training may also be provided in PICU and Acute Medicine specialities. Each doctor will be assigned an experienced Educational Supervisor to help plan their educational journey. A limited study budget will be available for suitable external training courses.

The post will have ample time for clinical audit, research, leadership and teaching. The appointee will be expected and encouraged to take part in these activities as part of their fellowship.

The trainee will have a huge amount of flexibility in regards to their work pattern. Whilst the on call activity will be based on the ICU, there is scope for time to be spent within the theatre environment or appropriate medical specialities should this be deemed appropriate for the appointed trainee. The post very much allows for the development of a trainee-directed programme that will be bespoke in nature.

Regular teaching sessions are held covering core topics based on the FFICM curriculum. There is the opportunity to attend M& M meetings, critical incident reporting and journal clubs.

Job responsibilities

The Critical Care Units in Leeds Teaching Hospitals are based in Leeds General Infirmary (LGI) and St James's University Hospital (SJUH). We provide critical care support for a comprehensive range of adult specialties. Across the two hospitals there are over 80 critical care beds, with plans for further expansion.

MEDICAL STAFF IN THE DEPARTMENT

The Critical Care team consists of consultant intensivists from anaesthetic and medical backgrounds, supported by ICM trainees, Junior Clinical Fellows, IMTs, ACCS/DRREM trainees, Respiratory StRs, as well as anaesthetic trainees.

In addition to this we are supported by a team of Advanced Critical Care Practitioners, critical care outreach nurses, and a highly skilled and dedicated nursing workforce.

St James's Hospital

Critical care is provided across two critical care units. Duties comprise all aspects of the critical care management of general medical and surgical patients, as well as patients from the regional specialist units on this site:

Liver surgery and Transplantation

Thoracic surgery

Regional respiratory and renal services

Pancreatic surgery

Haematology and Oncology

Infectious diseases

In addition to the many general training opportunities available in such a large critical care department, there will be specific opportunities for training and skills development in the following:

Management of acute liver failure and the liver transplant patient including pre and post operative care. Time can be allocated to training within the wider liver unit as required.

Critical care ultrasound in all FUSIC modules and echocardiography training to level 2 accreditation (FUSIC HD, BSE ACCE, fTOE / advanced TOE)

Advanced renal management including renal transplantation medicine and replacement therapy techniques, including regional citrate anticoagulation and intermittent haemodialysis.

Management of complex thoracic patients

Anaesthesia and advanced airway training for single-specialty critical care doctors

Training in teaching, management and research across a variety of settings within the trust.

Leeds General Infirmary

Critical care provision at the LGI is divided between neurointensive care, cardiothoracic, general and paediatric units. Regional expertise is provided in the following areas:

Major Trauma

Cardiac Surgery

Neurosurgery

Vascular surgery

Head and Neck surgery including major cancer surgery

Orthopaedics and plastic surgery including major reconstructive surgery

Paediatrics and paediatric intensive care

LEVELS OF RESPONSIBILITY

The appointee will be the senior non-consultant grade doctor working in the intensive care unit, with the same responsibilities as the advanced-level trainees in Intensive Care Medicine. As such, the postholder will be expected to work at a senior level, providing support to more junior members of the medical team and to the Advanced Critical Care Practitioners. The trainee will be the senior of 5 resident medical workforce out of hours.

TEACHING AND RESEARCH/STUDY AND TRAINING

The appointee will have regular input into the departmental teaching sessions and will attend educational sessions targeted at those trainees on the advanced ICM trainee programme. Opportunities for involvement in research projects are available, and the development of a specialist skill, such as FUSIC accreditation, is strongly encouraged.

The post is in line with the new junior doctor contract. This post is fixed term in order to provide a period of training. The post will consist of night and weekend on call commitment. There are five rotas staffed by the medical workforce which is further supported by the consultant team.
