If yes, we are offering a great opportunity to join our Swindon Dementia Advice team, providing personalised support to people diagnosed with dementia and their carers

About the role

Location: This is a homebased role which will require travel within the Swindon area.

Support will be provided in person, over the telephone, virtually (Teams), or in writing (emails). Our goal is to reach out to people affected by dementia from all communities and support their needs within their own community.The service will identify their needs and the services they may require, support the improvement of their overall well-being, and enable them to take control of their lives. We work closely with the local memory service and other health and social care partners to ensure the most appropriate support services are involved.

About you

This is a great opportunity for someone who is enthusiastic, committed, organised, and resilient, and who has experience or an interest in supporting people with dementia and their carers.

To be successful in this role, you will; 

- Have excellent communication skills as well as strong organisational skills to manage your caseload of clients effectively. 
- Be good at developing relationships and networking internally and externally with professionals and organisations in Swindon.
- Be empathetic and non-judgmental in your approach, committed to equal opportunity.
- Be passionate about putting people affected by dementia, their carers and supporters at the heart of everything you do to make our service the best it can be.
- Have a sound knowledge of IT systems such as emails, Teams and basic excel and experience of updating confidential personal records.
- Manage your time effectively. 
- Be able to build effective team relationships even though the role is homed based involving remote working.
- Be able to travel independently across the Swindon area, with occasional travel outside of Swindon for face to face area meetings.
