We are looking for Social Workers who are enthusiastic and passionate about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference and enabling them to increase their independence and have fulfilling lives, free from harm and abuse.

We currently have vacancies in the following teams:

  • Peripatetic/Continuing Health Care Team – Permanent, working 37 hours per week.  As a member of the Peripatetic / CHC Team, you will be expected to have a good understanding of the Continuing Health Care Framework.  You will assess the needs of adults, attend CHC/DST meetings and be able to effectively challenge CHC decisions. It is also expected that you could be deployed to other adult social care teams for short periods of time when support is needed.
  • Community Older Person’s Team – Permanent, working 37 hours per week.  Working in a busy and supportive team, this role will require you to support, safeguard and assess the needs of older adults over the age of 70, people with dementia and their carers.
