The fellows are an integral part of the cardiac surgical team, which apart from the Consultants comprises an SpR rotating from the Bristol Royal Infirmary and 2 other fellows. Being a small team, it is essential that there is good communication between the members of the team and that all care is provided considering the other team members.

The post is intended for a senior cardiac surgical trainee who has the intention of pursuing a career in congenital cardiac surgery and exploring a career with an academic component. It is anticipated that the applicant has completed their adult cardiac training and is entirely comfortable with all elements of adult cardiac surgery, pre-operatively, intra-operatively, and post-operatively. As such, trainees should have FRCS or equivalent. Training will be given in the practical techniques, in pre-operative assessment and post-operative management of the patients. Participation in operative procedures will be dependent on previous levels of experience and skill. The responsibility of ensuring a patient is ready for theatre lies with the fellow. Whilst post-operative care on PICU is primarily provided by the intensive care team, the successful candidate is expected to contribute significantly to this and to be first 'on-call' for postoperative patients.

The post includes an internal cover arrangement the other fellows will be required to cover their colleagues' absence by swapping 'on call' duties.

Main duties of the job

During the post, the post-holder will be expected to assimilate the following knowledge and skills. It should be emphasised that this list is not exhaustive and represents the minimum level of skills that we hope to impart. Knowledge and understanding of:

The intensive care environment, philosophy, differences from standard care, criteria for admission. Disruption of normal physiology in multi-organ involvement in critical illness The pharmacology and indications for commonly used inotropic/induction/anaesthetic/sedative agents. Normal physiology of the heart and circulation. Disruption of the normal physiology in congenital heart disease and following commonly performed cardiac surgical procedures. Circulatory monitoring and support Different modes of ventilation in children, including high frequency, extracorporeal systems and pulmonary vasoactive agents. The mechanisms of normal cerebral homeostasis and its disruption in critical illness/injury. Psychological support for children/family with critical conditions, including bereavement counselling.

Weekly Cardiac Educational Programme according to the week of the month this concentrates on:

Echocardiography Journal Club Specific Topic Teaching Cardiac Morbidity and Mortality Meetings Pre-surgical Conference This is a major teaching opportunity. Along with reviewing the cases for the coming week, new cases will be discussed.

Occasional guest Lectures Institute of Child Health

Training needs and levels of supervision are individualised to the trainees requirements. In addition:

An allocation for study and research and attendance at organised teaching sessions during the working week is allowed for, although study leave and expenses are discretionary for Trust funded posts. The post will have a named supervisor Medical libraries are located in the Bristol University Medical School- 2 minutes walk from BRCH and the Institute of Child Health The UH Bristol Education Centre accommodates several Trust departments, including Post Graduate Medical Education, Clinical Skills Centre, Resuscitation Services, Learning Resources Centre, Bristol Stimulation Centre and the Institute of Child Health. The Education Centre also offers a wide range of training facilities for both formal lectures and group work.
