QSW - Adults - Social Worker - Ageing Well West Team Objective: Recognise and build on each person’s strengths and desires to help them live as independently as possible.
Assume that individuals know what’s best for their own life goals and well-being.
Conduct assessments and help create personalised care plans that leverage local community resources, personal networks, and services to lessen reliance on formal support.
Collaborate with health and social care professionals, and other partners, to adopt creative and forward-thinking practices aimed at improving life in Southend.
Set an example of high-quality social work practice with excellent communication, knowledge, values, and a focus on the individual.
Encourage critical thinking and group work to maintain a safe, responsive, flexible, and emotionally intelligent work environment.
Planning and Organisation:
Work with partners, including the police and health and social care providers, to coordinate efforts and resolve any arising risks or issues.
Foster an environment that encourages innovative and bold practices to achieve the best outcomes for the community.
Promote a culture of active engagement and co-production within the team, community, and broader system.
Get involved in community engagement to combat isolation, loneliness, and disconnection amongst adults and families.
Apply critical thinking to the development and maintenance of services with community and stakeholders.
Build effective relationships with key stakeholders, such as GPs, primary care professionals, hospital staff, and community leaders, to contribute to a better Southend.

Take on professional duties for Deprivation of Liberty Safeguards, safeguarding enquiries, and Mental Capacity Act assessments when needed.
Manage your workload effectively, prioritising tasks to maintain high-quality service delivery.
Invest in your professional growth to handle the complexities of social work with confidence.
Be a proactive agent of change, supporting practices that empower and develop the community.
Pursue leadership training to enhance your ability to lead change and contribute to Southend Borough Council’s evolution into a learning organisation.
The selected individual should be prepared to work beyond standard hours, which may include weekends, bank holidays, and participating in an on-call rotation as necessary.
Flexibility to serve across different Social Work teams is expected, contingent upon the evolving needs of the service.
