Full job description

With respect to practice rotas, as agreed, the post-holder will make themselves available to undertake a variety of practice duties, including face-to-face consultations, telephone consultations, and rare home/care home visits. Duties include actioning bloods, checking and signing EPS repeat prescriptions and PA scripts, and dealing with queries as part of the day-to-day schedule. The post-holder will make professional, autonomous decisions regarding presenting problems, whether self-referred or referred from other health care workers within the organisation. Responsibilities include:

  1. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  2. Developing care plans for patients with long-term conditions.
  3. Providing counselling and health education.
  4. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
  5. Prescribing in accordance with the practice prescribing formulary and working to ICB guidelines.
  6. Supporting/supervising Physician Associates when required to ensure the smooth running of the practice.
  7. Managing and supporting ongoing palliative care and elderly care case loads.
