  • Collaborate with healthcare professionals to develop and implement individualized medication therapy plans
  • Provide medication counseling to patients regarding proper usage, potential side effects, and drug interactions
  • Monitor patients' responses to medication therapy and make necessary adjustments to optimize outcomes
  • Participate in interdisciplinary rounds and provide clinical recommendations to the healthcare team
  • Conduct medication reconciliation to ensure accurate medication histories for patients
  • Stay updated with the latest advancements in pharmacotherapy and contribute to evidence-based practice initiatives
Requirements
  • Pharmacy Degree MRPharmS
  • Registered with GPhC
  • 2 years NHS Experience
  • Post graduate experience in hospital pharmacy
  • Experience in working in an out of hours service
  • Experience of working on general medical wards, stroke and elderly care wards
  • EPMA (Cerner or other software) experience
