About you:
As an Occupational Therapist, you will provide occupational therapy within the care home managing a defined caseload, using evidence based, person centred principles to assess, plan, implement and evaluate interventions in an environment as agreed by home management and regional team. _*Whilst the role will be based at our care home in Rhayader, flexibility will be required to assist with other homes within the region.*_

*Responsibilities include\**
· To undertake occupational therapy assessments for a designated caseload, assessing occupational performance of our residents, enabling the resident in areas of independence, productivity and leisure/wellbeing
· Through co-production with residents develop occupational therapy goals as part of their care plan
· Plan and implement individual and group interventions, in collaboration with the residents, using graded activity to achieve therapeutic goals
· Monitor and evaluate the treatment plans in order to measure the progression and effectiveness of the care plan outcomes
· Effective communication networks with residents and nursing team within the care home as well as developing those relationships with external health care professionals within the community
· To participate as part of the in-house multidisciplinary team, contributing to decisions and resident care planning
· Support and educate residents and staff within the care home regarding aspects of occupation therapy including rehabilitation programs, ensuring families are kept informed of plans in place
· Actively participate in formal and informal reporting including the documentation on the care homes care planning software
· To contribute to the company clinical care department relating to clinical governance arrangements and quality assurance
· To maintain understanding and knowledge of national guidelines and legislation relating to health and social care and the provision the care home provides.
· As directed by your line manager, to participate in clinical effectiveness and clinical audit procedures, interpreting the outcome and relate to the practice of occupational therapy as part of the quality assurance process
· To carry out and assist the home in risk assessments to guide practice, resident safety and the safety of staff
· Demonstrate and critically evaluate current research and apply this to your practice within the care home, ensuring the most current and best practice is in place
· To maintain a professional portfolio to count towards CPD, recording learning outcomes through participation in internal and external development opportunities
· To be involved in the planning, evaluation, clinical pathways and protocols within your care home / region
· Using IT skills to produce reports and contribute to care planning using an electronic care planning software
· To exercise good personal time management, punctuality and consistent reliable attendance

*Essential Requirements:*
· ·Diploma / Degree in occupation therapy
· HCPC registered
· Effective written and oral communication skills
· Good personal organisation skills
· Ability to travel within a geographical area
· Able to work hours flexibly
· Clinical practice in variety of general and specialist areas
· Proficient in computer / IT skills
· Ability to make independent, sound, clinical decisions and problem solve.
· Ability to manage busy case load and prioritis
· Ability to work autonomously using specialist knowledge, skills and initiative.
· Maintain confidentiality and professionalism
*Desirable:*
· Additional clinical or healthcare related experience
· Knowledge of principles of clinical governance
· Ability to speak Welsh
· Agored assessor
· Ability to reflect and critically appraise own performance
· Experience of working within a care home
