Support Coordinator We're looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in Fylde Coast area. This is an exciting opportunity to work with stroke survivors and their families to support them following stroke.
Position: S11175 Stroke Support Coordinator
Location: Home-based, Fylde Coast, however, regular travel will be required as part of this role (May include team meetings or other work-related meetings)
Hours: Part-time, 21 hours per week
Contract: This is a fixed-term contract until 31 March 2025. Services are contracted and there is currently funding for this contract until 31 March 2025.

Interviews will be held via video conferencing. The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.
A key part of the role will be to recruit, train and manage a team of volunteers, to support service users and the delivery of the service.
The Stroke Support Coordinator will:
Support new stroke survivors and their carers from hospital discharge into the community.
Provide personalised information, advice and support.
Support clients to make informed lifestyle changes which will help them to prevent further strokes
Be accountable for volunteers, so experience of recruiting, training and managing volunteers would be an advantage.
A caring profession ideally with experience of supporting people with disabilities.
Setting up and running support groups.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Recovery is tough, but with the right specialist support and a ton of courage and determination, the brain can adapt.
The charity provides specialist support, funds critical research and campaigns to make sure people affected by stroke get the very best care and support to rebuild their lives.

And particularly applications from those with lived experience of stroke and those from under-represented communities.
