The ideal candidate will have a strong background in social work, excellent communication skills, and a commitment to enhancing the quality of life for vulnerable adults.
Assessment and Care Planning: Conduct comprehensive assessments of adults' needs, risks, and strengths. Develop and implement personalized care plans to address identified needs.
Provide practical and emotional support to adults, helping them to achieve their goals and improve their quality of life.
Safeguarding: Identify and respond to safeguarding concerns, ensuring the safety and well-being of vulnerable adults.
Collaboration: Work closely with other professionals, including healthcare providers, housing officers, and community organizations, to coordinate and deliver comprehensive support.
Advocate for the rights and needs of adults, ensuring their voices are heard and their choices respected.
Maintain accurate and up-to-date records of all assessments, interventions, and decisions, ensuring compliance with relevant policies and procedures.
A degree in Social Work and current registration with Social Work England.
Experience: Previous experience working with adults in a social work setting.
Ability to build positive relationships with adults and their families.
Knowledge of relevant legislation and best practices in adult social care.
Commitment to safeguarding and promoting the welfare of vulnerable adults.
