Do you have proven experience implementing or supporting Patient Safety Incident Response Framework (PSIRF) with demonstratable experience in conducting or overseeing patient safety investigations?
As the Patient Safety Manager, you will work in the support of the Group Quality Assurance Director, with the patient safety team to ensure effective working between regional patient safety, and the corporate patient safety functions, ensuring compliance with Elysium PSIRF Policy while supporting a culture of learning and continuous improvement.
You will oversee patient safety investigations, in line with PSIRF, providing support, guidance and training to investigating officers. Acting as a conduit between internal teams and external stakeholders, the position ensures compassionate engagement, strengthens system resilience, and promotes best practice in patient safety, risk management, and quality assurance.
You will also be required to deputise for the Group Quality Assurance Director.

This is a remote-based role with regular travel to our services across the UK
As a Patient Safety Manager, you will:
Patient Safety Management:
Support the daily management of patient safety processes and effectively manage the investigation of patient safety incidents across Elysium in line with the patient safety policy and PSIRF.
Raise risk concerns to Group Quality Assurance Director and the Patient Safety Team.
Work with the legal team to support the process of mortality review and other legal matters, as required.
Oversee thorough investigations into patient safety incidents using PSIRF
Using SEIPs model, provide support to those undertaking patient safety investigation.
Thoroughly review responses to patient safety incidents, suggest adjustments to plans or strategies, and request further evidence as necessary.
Analyse and review escalated Patient Safety Events via P1 and P2 reports at weekly meetings, including assessing appropriate learning responses.
Review Patient Safety trend information in collaboration with the Patient Safety Team.
Obtain specialist advice when required from the Patient Safety Team.
PSIRF Implementation & Learning:
Support services to implement and deliver PSIRF and collaborate with relevant services and clinical networks to embed learning from patient safety events.
Support the delivery of PSIRF-related training.
When required, chair and facilitate PSIRF Quality Summits.
Have oversight and provide support to investigators, review final reports, and support quality checking prior to circulation.
Support the patient safety team with collation of information for reports required internally and externally.
Provide and receive complex, sensitive, and contentious information and present as appropriate to the patient safety meeting.
Contribute to regular reports from the Patient Safety Workstream to Clinical Corporate Governance, the Elysium Management Board, Risk Committee, and Ramsay Healthcare.
Support the development and timely delivery of all Elysium board reports relating to Patient Safety and Mortality.
Establish a culture of teamwork and cooperation within the patient safety domain.
To undertake supervision, to actively support and encourage team members.
Proven experience in implementing or supporting Patient Safety Incident Response Framework (PSIRF) or related patient safety, risk, or governance frameworks
Demonstrable experience in conducting or overseeing patient safety investigations
Experience in engaging with service users, carers, or staff to support learning and improvement following patient safety events
Experience coordinating multidisciplinary teams in quality, safety, or risk management initiatives
Experience in analysis of patient safety data, trends, and the development of improvement action plans
Experience with NHS or independent sector mental health, learning disability or hospital environments
Experience in delivering training or facilitating workshops on patient safety, incident management, or governance topics (D)
Evidence of continued professional development in patient safety, risk management, governance, or quality improvement
Training in investigation methodologies and/or significant experience in leading incident investigations
